Please note that upon booking a confirmation email will be sent to your chosen email address.

It is essential that all patients read the confirmation email for their Initial Assessments in full as we require forms to be completed prior to your appointment. Failure to complete these forms may mean that your treatment time is impacted and patients will still be required to pay for the full appointment as booked.
